$22.8 million on knees in three months: what New Zealand's health insurance claims really pay for
Southern Cross's claims data for July to September 2026 shows where health insurance money actually goes, what one operation costs, and why age changes everything.
In just three months, July to September 2026, Southern Cross Health Society paid $107.5 million for orthopaedic claims alone. Its latest claims update shows what New Zealanders actually claim for, and it is not always what people expect.
The five biggest areas of spending
- Orthopaedic: $107.5M across 6,146 claims
- Imaging and tests: $58.6M across 152,654 claims
- General surgery: $44.7M across 4,727 claims
- Specialist consultations: $44.3M across 175,891 claims
- Gynaecology: $40.7M across 4,269 claims

The everyday claims add up
Scans, tests and specialist visits together made up more than 328,000 claims in the quarter. Many people think of health insurance as 'surgery cover', but the first step is usually a specialist and a scan. If your policy covers surgery only, you may be paying for those first steps yourself.
What one operation costs
Dividing Southern Cross's totals by the number of claims gives a rough average per claim:
- Total knee replacement: roughly $32,000 (708 claims, $22.8M)
- Total hip replacement: roughly $30,000 (682 claims, $20.5M)
- Hysterectomy: roughly $32,000 (444 claims, $14.2M)
- Spinal fusion: roughly $74,000 (226 claims, $16.8M)
- Colonoscopy: roughly $2,800 each, and 7,648 of them ($21.7M)
These are averages, and individual costs vary. The question worth asking is simple: if you needed one of these tomorrow, would you have that amount in savings?
Age changes everything
The average amount paid per claiming member was $1,095 for ages 20 to 34, $2,324 for ages 50 to 64 and $3,344 for those 65 and over, about three times as much as the younger group. Members aged 65+ received $195M of the quarter's payouts, the largest share of any age group.

That is exactly when cover matters most, and also when premiums tend to be highest. Keeping good cover affordable into your 60s takes planning while you are younger.
What to check in your policy
- Is your surgical limit high enough for a joint replacement or spinal surgery?
- Are specialist visits and diagnostic tests included, or is it surgery-only?
- Is your excess still right for your budget?
- Is your premium structure set up to stay affordable as you age? Our Premium Check page can help you see the difference.
